Fire origin and cause investigation of a manufacturing plant

Summary

Our experts were asked to investigate an explosion that occurred at a manufacturing plant being refurbished with a new refrigerated cooler. While welding pieces of the equipment together, an explosion occurred. Experts for the installer reported that the cause of the explosion was the result of a blocked sewer line and accumulation of methane gas. EFI Global responded by dispatching an engineer and chemist to determine the facts and cause of the event.

The investigation determined no connection between the sewer lines and drains where the explosive gases were alleged to have been generated. Furthermore, it was determined that the condensate drains had been taped by the installer which impeded ventilation. The heat generated during the welding operation on the cooler caused styrene gas to form and accumulate in the airspace. After conducting laboratory testing of the products involved, our chemist found that styrene gas was produced during the heating of the expanded polystyrene (EPS). The next strike of the welder caused ignition.
